For the purposes of DSP, SA and determining partial capacity to work (1.1.P.56), medical evidence is documentation that provides information about a person's medical condition(s) (1.1.M.90). This documentation may include a report from the person's treating doctor, x-rays, a pathology report or a report from health professionals (1.1.H.50) including para-professionals.
Diagnosis of a medical condition that may qualify a person for DSP can only be provided by a registered medical practitioner (with limited exceptions).
Explanation: The primary source of medical evidence is a report from the person's treating doctor. Other documentation mentioned above would be used to supplement information provided by the treating doctor (1.1.T.160) or other medical practitioners.

For the purposes of CP (child), under the extension of short term (1.1.S.172) or episodic care (1.1.E.132) provisions, medical evidence is a certificate from a THP which confirms that due to the care receiver's disability or medical condition (which must be the same or related to the original disability or condition) they need a further period of personal care which continues beyond the period initially advised.
